By participating in the survey you will help us better meet the NPDB mission of improving health care quality, protecting the public, and reducing health care fraud and abuse in the U.S.

If you have not received an invitation and wish to participate in the survey, please send an email to NPDBSurvey@hrsa.gov. The survey is open from May to June 2021.
