Welcome to the Self-Query Service

A self-query is a request by a practitioner, provider, or supplier for information about himself, herself, or their organization contained in the NPDB and the HIPDB. A self-query may be performed at any time, and must be submitted through this on-line self-query service.

Perform a Self-Query
Go to the Self-Query Service

Complete the required information on the Self-Query Input screens and then print a copy of the generated self-query. Sign the formatted copy (in ink) in the presence of a notary public and mail the notarized form to the NPDB-HIPDB. You may find it helpful to print the Helpful Hints for Submitting a Self-Query to use as a reference when completing the application.
